What is The Safety Net Foundation?

The Safety Net Foundation is a nonprofit foundation supported by Amgen that provides Amgen products at no cost to qualifying patients with no or limited drug coverage.

How do patients apply for assistance from The Safety Net Foundation?

To enroll in The Safety Net Foundation, patients must complete the Patient Application Form and meet program eligibility requirements.

For replacement model products, the patient's provider is responsible for applying to the Foundation on their behalf. The provider must also be enrolled in the Foundation before they can enroll a patient.

For prospective model products, the patient can apply directly to the Foundation. Providers do not need to be enrolled in the Foundation for prospective products.

What are the eligibility criteria for The Safety Net Foundation?

To be eligible for The Safety Net Foundation, you must be a U.S. resident, demonstrate financial need, and have no or limited drug coverage. Qualifying Medicare part D patients may also be eligible.

The Safety Net Foundation income requirements are based on a percentage of the Federal Poverty Level, adjusted for household size.

How long are patients enrolled in The Safety Net Foundation?

Eligible uninsured patients are typically enrolled in The Safety Net Foundation for one year, starting from the date the Foundation makes an enrollment decision. Eligible underinsured patients are typically enrolled through the end of their plan year.

I don't have insurance. Can The Safety Net Foundation help me access Amgen medicines?

Yes, if you meet the eligibility requirements, you may qualify for Safety Net Foundation support and receive certain Amgen medications at no cost.

Maybe. The Safety Net Foundation supports underinsured patients (including Medicare Part D) who have limited coverage for Amgen medications in certain situations.

Does The Safety Net Foundation help with all settings of care?

The Safety Net Foundation is available only to eligible patients treated in the outpatient setting. The outpatient setting is defined as any physician's office, hospital outpatient department, or pharmacy setting.

What is the prospective model?

The prospective model applies to products that require a product prescription and delivery of medication in advance of administration. Under this model, providers complete a product prescription form, or submit an orignial script to the Foundation for the applicable Amgen product. Foundation products included in the prospective model are Prolia® (denosumab) injection for bone health and Sensipar® (cinacalcet). The Safety Net foundation will either ship prospective product directly to the patient, or to the provider's office on behalf of the patient.

What is the product replacement model?

The replacement model applies to physician-administered medications that are covered under the patient's medical benefit (versus pharmacy benefit). Under this model, providers administer Amgen product from their existing stock to qualifying Foundation patients and then order replacement for this product from the Foundation. Foundation products included in the replacement model are Aranesp® (darbepoetin alfa), EPOGEN® (Epoetin alfa) for dialysis only, Neulasta® (pegfilgrastim), NEUPOGEN® (Filgrastim), Nplate® (romiplostim), Prolia® (denosumab) injection, Vectibix® (panitumumab) injection, and XGEVA® (denosumab). These products must be administered in an outpatient setting to be eligible for replacement. Prolia® is shipped directly to the provider.

Does the replacement model cover product administered before the patient becomes enrolled?

Yes, the replacement model allows for a six-month retroactive policy for replacements. Once a patient becomes enrolled, replacement product may be requested for dates of administration up to six months prior to the patient's enrollment start date.

What is the product replacement request form?

The product replacement request form is the document that enrolled facilities/providers use to request product replacement for enrolled Foundation patients. Providers can submit multiple product replacement requests for multiple patients and products on one form.
